Prompt Report Form
What is this form?
A Prompt Report Form is used to document and report unexpected animal welfare concerns, protocol deviations, or noncompliance events to the Institutional Animal Care and Use Committee (IACUC). All events or concerns that require prompt reporting should be submitted electronically through the Access Portal to the Office of Research Compliance.
Prompt-reporting is a critical component of a strong animal care and use program and fosters a culture of ethical research. When investigators and animal care personnel promptly report protocol deviations, adverse events, unanticipated animal health issues, or potential noncompliance, the institution can rapidly assess animal welfare concerns, implement appropriate corrective actions, and reduce the likelihood of recurrence. Timely prompt-reporting demonstrates accountability, transparency, and a shared commitment to humane animal care and regulatory compliance.
Ultimately, prompt-reporting promotes animal welfare, preserves the integrity and validity of the research, and strengthens the institution's credibility with regulatory oversight agencies and the public. Use this form if there is an incident related to live vertebrate animals that are involved in:
- Research
- Teaching
- Training
- Teaching
- Experimentation/Testing
Have the following ready, if available:
- Whether the issue/event is related to:
- a single protocol
- multiple protocols
- or none
- Protocol number(s)
- PI email
- Study title
- Date(s) of the issue/event
- Building/location
- Department and room number
- Click Access Portal above
- Click Submit New Form
- Complete the required fields
- Select all applicable issue categories related to the incident
- Describe the event or concern in detail
- Click Submit
- The IACUC Office will receive your form
- You can track your submission in the Submissions page
