For students seeking accommodations on standardized examinations for a licensing or certification board (e.g., Texas Examination of Educators Standards (TExES) exam series, Association of Social Work Board (ASWB) licensure exam, Certified Rehabilitation Counselor (CRC) certification exam, Physician Assistant National Certification Exam (PANCE), etc.), please complete the form below and click the "submit" button at the bottom of the page. Once submitted, please allow 10 business days for a response from Student Accessibility Services (SAS).
Potential delays in processing the request by SAS may occur if:
- An incomplete form is submitted by the student.
- SAS records have not been updated with current documentation since the initial SAS application, thus SAS staff are not able to validate the request.
- Documentation requires a qualified professional to describe or diagnose the disability.
- Each board identifies a documentation currency policy and SAS is required to adhere to it.

In addition, SAS is not responsible for meeting deadlines of licensing or certification boards. Rather, the requestor (i.e. student) must submit the SAS form below with adequate time to process the accommodation request. Students should review their upcoming exam schedule with advance notice of a board's deadline.
