The Fitness to Teach Referral Form shall be used to document any serious dispositional concern involving a student. Minor concerns such as missing class sessions should be addressed through EAB Navigate in accordance with established University procedures.
Individuals submitting the Fitness to Teach Referral Form are expected to provide a clear, objective, and comprehensive account of the concern, including relevant observations, factual information, supporting documentation (when available), and any actions taken to address the issue. All information submitted will be reviewed by the appropriate Dispositional Review Committee as part of the Fitness to Teach process. The documentation may be used during meetings with the student and, when warranted, will serve as the basis for the development, implementation, and monitoring of a Growth Action Plan.
