Test Score Retrieval Request

Test Score Retrieval Instructions

Request Processing Time: Score retrieval requests will be ready for pick up within 24 hours of the request. Requests submitted over the week end will be processed on the next business day. A valid photo ID is required to pick up score reports, and the name on the ID must match the name on the score report.
Complete the Form in Full: Please complete all required fields on the form. Requests with missing or incomplete information may not be processed within the 24-hour timeframe.
Test Date Information: The test date is required. If you are unsure of the exact date, please provide an approximate timeframe
Select the Correct Exam: Be sure to select the correct exam for which you are requesting scores
Notification: A Testing Center staff member will contact you via email once your score report is ready for pickup
