BSN Course Requirement Self Evaluation Worksheet

Generic and 2nd Degree BSN Applicant Checklist

Date: 02/25/2026

For Applicant Use Only — DO NOT UPLOAD WITH APPLICATION

UTRGV School of Nursing Undergraduate Programs

All Generic and 2nd degree Applicants must receive the Verified Status.

My BSN Application Cycle Start: __________________

Application Cycle Closing Deadline: __________________

Start working on your application as soon as the cycle is open. Order official transcripts 5 weeks before the closing deadline for NursingCAS directly from the application, and submit your application 2–4 weeks prior to the closing date to troubleshoot any issues before the deadline and ensure Verification status is received by this deadline or no later than the grace period for resolving any Verification issues.

The UTRGV School of Nursing has extended the application window from 30 days to 60 days. Use this additional time to begin and complete your application early. Submitting during the final week or two weeks may prevent applicants from resolving issues identified by NursingCAS during the Verification process. Applicants who submit later than the timeline above risk missing the Verification deadline—especially if they have not followed all instructions, such as completing full transcript entries for every college or university attended or ensuring that NursingCAS receives all required official transcripts to begin Verification before the application closes.

Completion of this checklist does not guarantee admission to the nursing program.

Application Overview

  • Two applications are required:
    • One application for UTRGV via ApplyTexas.org six weeks before the cycle opens but no later than the first two weeks after the cycle is opened.
    • One application for the School of Nursing via NursingCAS.
  • Two sets of official transcripts are required to be ordered by the applicant:
    • One set of official transcripts must be ordered for NursingCAS directly from the portal for all colleges or universities you attended regardless of whether the credits apply to the nursing program, including your UTRGV transcript.
    • One set of official transcripts must be ordered and sent to UTRGV Undergraduate Admissions for all colleges and universities where you took courses and earned any of the 63 required credits. Transfer credit notations on other college or university transcripts cannot be utilized.

Step 1: Apply or Reapply to UTRGV via ApplyTexas.org and Submit Official Transcripts to UTRGV

  • _____ I have applied or reapplied to UTRGV via ApplyTexas.org and have submitted all official transcripts to UTRGV Undergraduate Admissions from all institutions where I earned any of my required prerequisite credits at least six weeks before the cycle opened as recommended, but no later than the first two weeks of the cycle being opened. I have been accepted to UTRGV.

Important: All applicants must have submitted all official transcripts and be accepted to UTRGV by the program’s application closing deadline, or their application will be incomplete and not considered.

  • Steps to apply to UTRGV
  • Contact Undergraduate Admissions
  • _____ I understand that if I am currently enrolled as a first-degree seeking student who is not graduating with a bachelor’s degree or who will not have a degree before the nursing program’s start date, I do not need to apply or reapply to UTRGV. My major will be changed if accepted to the BSN Generic program.
  • _____ I understand that if I am a current UTRGV student in my last semester and will be graduating with a bachelor’s degree, I need to reapply to UTRGV before completing my last term after completing my Application for Graduation.

Important: Applicants with a bachelor’s degree or who will have one by the program start date should apply for the ABSN (2nd degree BSN) program.

  • _____ I understand that if I am a returning UTRGV student who has missed or will have missed a full Spring and Fall term by the time the program starts, my UTRGV application will not be active, and I must reapply to UTRGV. I will contact UTRGV Undergraduate Admissions if I have any questions.
  • _____ I understand that UTRGV Undergraduate Admissions must evaluate my Credit by Exam credits or scores. I will follow their instructions to submit any Credit by Exam scores for evaluation and contact Admissions if I have any questions. I understand that I must submit an official transcript to NursingCAS with any Credit by Exam credits I have completed.

Important: If any exams are in progress or planned to be taken after the application is submitted, the applicant must immediately submit scores to Undergraduate Admissions and complete an Academic Update in NursingCAS once the scores are reflected on an official transcript. Send a copy of the scores and unofficial transcript to schoolofnursing@utrgv.edu so the application information can be updated.

  • _____ I understand that I must receive an email confirming my UTRGV acceptance before the nursing program’s application closing deadline. I will follow up with Undergraduate Admissions if I do not receive this email within two weeks of applying to UTRGV.

Step 2: Apply to the School of Nursing via NursingCAS

Recommended timeline: Submit 2–4 weeks before the application closing deadline and order official transcripts 5 weeks before the application closing deadline.

Contact NursingCAS Help Center regarding any application questions, not the School of Nursing. Use their search bar to find guides and answers to application questions.

  • _____ I understand that I can review the NursingCAS Help Center to find step-by-step guides and videos that will help me complete my NursingCAS application.
  • _____ I understand that I must contact NursingCAS, and not the School of Nursing, if I have any questions regarding my application and the Verification process. The School of Nursing does not monitor or review applications until they are verified by NursingCAS.
  • _____ I understand that the NursingCAS “Supporting Information” section with achievements, licenses, references, and similar items is not utilized or required, and applicants are to skip that section for this application.
  • _____ I understand that I must order official transcripts from all colleges or universities where I earned any credits, including UTRGV, five weeks before the application closing deadline for NursingCAS, regardless of whether the credits apply to the nursing program. I will ensure there are no holds on my records that would prevent official transcripts from being sent.

Important:

  • Former UTPA students, as well as UTB/TSC and UTRGV students and alumni, must request official transcripts from UTRGV for NursingCAS.
  • UTB/TSC transcripts are generated independently from UTRGV/UTPA transcripts. Students should list UTB/TSC separately on the NursingCAS application and request that transcript separately.
  • All students who completed courses at STC, TSTC, or other colleges or universities must order official transcripts from those institutions for both NursingCAS and UTRGV.
  • _____ I understand that transfer credit notations cannot be evaluated by UTRGV for credit. The official transcript must come from the university or college where the credit was earned.
  • _____ I will ensure NursingCAS receives all official transcripts 2–3 weeks before the application closing deadline and follow up with my previous institution if they were not received.
  • _____ I have completed and submitted my School of Nursing application in NursingCAS 2 to 4 weeks before the application closing deadline to allow time for the Verification process and for me to resolve any issues before the deadline.

Important:

  • Applications must upload all valid HESI score documents, including Critical Thinking, which is on a separate score sheet, before the cycle closing deadline. Late scores are not accepted.
  • Applicants must read HESI FAQs and HESI Requirements and Information for important information about valid HESI scores and other requirements.
  • _____ I understand that NursingCAS states it takes about two weeks to reach Verified status, but it could take up to four weeks if my application has incorrect transcript entries, missing transcript entries, or missing official transcripts.
  • _____ I understand that I must check my NursingCAS portal and email every business day after submission of my application to ensure a Verified status is received. I understand that it is critical for me to review any notifications and statuses I receive and resolve them immediately with NursingCAS, not the School of Nursing.
  • _____ I understand the different statuses that my application will go through and will consult with NursingCAS if I have any questions or need to resolve any issues.

NursingCAS Application Statuses

  • Received = You have submitted, but documents are missing since applications must be verified. Take immediate action to resolve this and contact NursingCAS.
  • Complete = NursingCAS received official transcripts for reported colleges and institutions attended, and the application is in line to be reviewed for Verification. Monitor daily, as your application can be returned as undelivered after review.
  • Undelivered = NursingCAS started its review and found missing official transcripts, missing transcript entries, incorrect transcript entries, or other issues. Take immediate action to resolve this and contact NursingCAS.
  • Verified before the application closing deadline or within the 14-business-day grace period = SON will begin to process and review your application. The applicant must complete an Academic Update in NursingCAS if courses were completed after submission of the application.
  • Verified after the 14-business-day grace period = I missed the UTRGV School of Nursing final deadline and will need to reapply next cycle.
  • _____ I understand that submitting my NursingCAS application or ordering official transcripts after the prescribed timelines above is not encouraged and could reduce the time I have to fix any issues or errors or limit my ability to receive Verified status before the deadline or within the 14-business-day grace period.
  • _____ I understand that if my application is not Verified by the application closing date or within the 14-business-day grace period, it will be marked incomplete and will not be processed or considered. I also understand that I may reapply during the next application cycle. No further extensions will be granted for any reason, including transcript-processing delays caused by transcripts not being ordered according to the instructions and timeline.
  • _____ I will check my UTRGV email for any courtesy notification I receive from the School of Nursing after my application is Verified. I understand that applications with Verified status will be reviewed and processed for consideration.

Step 3: Verify You Have the Required Courses by Checking DegreeWorks, Your Transcripts, and Academic Record

  • _____ I completed all the following required application criteria courses with a minimum grade of C.

All the following courses must be completed before the application cycle closes. These are the application eligibility courses. If you have any in progress while the application cycle is open, they must be completed before the application cycle closes. A courtesy secondary review may be provided before the UAPG meets to make decisions if they are not, but this review is not guaranteed.

Science Courses

Science GPA = 40% of your rank score for your School of Nursing application

  • BIOL 2401 Anatomy and Physiology I
  • BIOL 2402 Anatomy and Physiology II
  • CHEM 1311 General Chemistry
  • CHEM 1111 General Chemistry Lab
  • CLSC 2429 Clinical Microbiology

Strategic Courses

Strategic GPA = 20% of your rank score for your School of Nursing application

  • MATH 1342 Elementary Statistical Methods or MATH 1343 Introduction to Biostatistics
  • PSYC 2301 General Psychology
  • PSYC 2314 Psychology of Lifespan
  • SOCI 1301 or ANTH 2351 Introduction to Sociology

You will not be a qualified applicant, and your application will be incomplete and not ranked or considered if the above required application criteria courses are not completed before the cycle closes. This also includes any Credit by Exam requirements that are not complete if the applicant is seeking credit by exam for the above courses.

  • _____ I understand that the application criteria courses above should not be in progress after the cycle closes. If they are completed after the cycle closes and the selection committee has not met to make selections, a courtesy secondary review may be provided, but this is not guaranteed.
  • _____ I have reviewed the 63 BSN credits that need to be completed for admission on the 2nd degree or Generic BSN Roadmap.
  • _____ I understand that a minimum cumulative 3.0 GPA is required for the BSN prerequisite courses for admission to the program and that SON does not utilize or consider an overall GPA.
  • _____ I have completed all required BSN prerequisites and core (63 hours) with a C or higher or will complete the admission criteria courses before admission to the program.
  • _____ If I am unsure whether a course may need a substitution, I will seek substitution review prior to cycle opening or no later than the first two weeks of the cycle being opened to allow time for review. BSN Generic applicants must meet with their academic advisor to review and request substitution approval from SON. 2nd Degree applicants only can contact the 2nd Degree BSN Program Coordinator.
  • _____ I understand that for any courses in progress at other institutions that are completed after submitting my application, I must submit my official transcript to UTRGV Undergraduate Admissions for evaluation and an unofficial transcript to schoolofnursing@utrgv.edu after I complete my courses in progress. I understand that I must also complete an Academic Update in NursingCAS.

Step 4: Check My HESI Requirements

Plan accordingly to take your HESI exam. Seats fill up fast at the UTRGV Testing Center, and you can only retest after 3 weeks. You may not submit HESI scores after the cycle closing date. You may want to take your HESI exam before the cycle opens to allow enough time to retest if needed.

  • _____ I made a minimum of 75 in each HESI component below and 750 for Critical Thinking is required.

HESI Exam: Your total cumulative score is 20% of your rank score for your School of Nursing application.

My HESI Cumulative Score is: __________________

Must have taken all HESI sections in one sitting, including any retests, if you would like a new cumulative score. Critical Thinking may be taken in a separate sitting, as that is not part of your cumulative score. The highest valid cumulative score submitted via score sheet will be used.

My Second Attempt HESI Cumulative Score is: __________________

Reminder: all components except Critical Thinking must be taken or retaken in one sitting to get a new cumulative score. Critical Thinking score is not part of your cumulative score, but it is required.

Required HESI Components

  • Grammar — minimum score of 75
  • Reading — minimum score of 75
  • Vocabulary — minimum score of 75
  • A&P — minimum score of 75
  • Math — minimum score of 75
  • Critical Thinking — minimum score of 750

Critical Thinking Score: __________________

Critical Thinking score is 20% of your rank score.

Review the Undergraduate College for Application Eligibility and Admission Requirements

  • _____ I uploaded a copy of all my official HESI scores, including my Critical Thinking score, to my nursing application. I understand that I may not take HESI and submit those scores after the cycle closes. My application will be incomplete and will not be considered if I have missing HESI scores.
  • _____ My HESI scores were not expired. Scores are only valid for one year from the date I took them until the date I upload them to my nursing application. My application will be incomplete and will not be considered if I submit expired scores.
  • _____ I did not take my HESI exam or any section less than three weeks apart. I understand that any section or exam taken less than three weeks apart is not valid and will not be considered for admission.
  • _____ I understand that I must take all components in each sitting if I wish to receive a new cumulative HESI score. Critical Thinking does not need to be retaken to obtain a new HESI cumulative score unless I scored less than 750. SON will take the highest valid cumulative score on the score sheet submitted.
  • _____ I understand that I can only take the HESI exam or any component twice in each academic year, from September 1 through August 31. A third exam taken at any college, university, or via distance testing service in an academic year is invalid and not considered, and I will need to reapply for another cycle.
  • _____ I understand that I may elect to retake and submit only HESI components for which I did not make the minimum score, but my first cumulative score will stand unless I take all components together in one sitting again. Critical Thinking does not need to be retaken for a new cumulative score unless I scored less than 750.
  • _____ I read the HESI Requirements and FAQs.

Helpful Application Tips

  • Prepare and plan in advance and do not wait until the last week or two to submit your application.
  • All BSN Generic applicants should seek advising at the Academic Advising Center to review their degree plan and ensure readiness to apply. Do not contact the BSN Program Coordinator for advising unless accepted to the nursing program.
  • All 2nd degree BSN applicants may be advised by the 2nd Degree Program Coordinator only if they have a bachelor’s degree or will have one from another university. Follow instructions on the Advising and Course Substitutions information on the SON website.
  • All applicants can attend a BSN information session each fall and spring term and speak to a SON BSN Program Coordinator.
  • The earlier you submit your application, the earlier you can troubleshoot issues and ensure Verification is received before the deadline.
  • Keep a calendar with due dates and set up alerts to ensure deadlines are met.
  • For detailed application instructions, read When and How to Apply.
  • For NursingCAS application assistance, review How to Apply and the Application Help Center for step-by-step guides and videos on how to complete each section of your application.
  • Contact NursingCAS via chat, email, or phone if you have any questions regarding your application through the Verification process. The School of Nursing is unable to address issues regarding completion of your application and the Verification process.
  • Applicants will receive notifications on the platform if there are pending NursingCAS issues that need to be resolved. If your application is returned and marked undelivered, check your status daily and correct any issues immediately with NursingCAS.
  • Read the HESI Requirements and FAQs.
  • Review the Testing Center’s HESI information and flyer.
  • Read How Applicants are Ranked.
  • See demographics of previously accepted applicants.
  • Attend BSN workshops offered each fall and spring term and BSN information sessions.

The BSN Applicant Checklist, including any application instructions and requirements, are subject to change. Check the SON website for the most current checklist. Applicants must follow the instructions on the NursingCAS application and complete all required sections, submit all official transcripts, and get Verified by NursingCAS according to the prescribed timeline and deadline.

NursingCAS Checklist 3/05/2026

Generic BSN, 2nd Degree BSN, and RN to BSN Programs

My Program Application Closing Deadline Is: ____________________________

Your complete application must be submitted a minimum of 2-4 weeks before your application closing deadline to allow time for the NursingCAS Verification process to be completed before the deadline and time for you to correct any issues.

All applicants must receive the Verified Status by the application closing deadline after submission of your application (see the 14-business-day grace period below). To accomplish receiving the Verified Status, utilize the time wisely and follow the instructions and timeline below. The application is open for at least 60 days, and that is sufficient time to receive the Verified Status.

Applications not receiving the VERIFIED Status by the application closing deadline (or within the 14-business-day grace period) are incomplete and will not be processed or considered.

No time extensions will be provided.

Applicants Must

  • Read and follow the detailed application instructions, requirements, and timelines to request official transcripts, submit application, and complete all required steps under When and How to Apply.
  • Read and follow all instructions on the NursingCAS application.
  • Utilize the Applicant Checklist on the webpage in addition to this checklist.
  • Utilize the NursingCAS Help Center, including the Get Started Guide, FAQs, and NursingCAS Timeline. These NursingCAS resources serve as a guide, but applicants must adhere to all School of Nursing application instructions, requirements, and timelines for ordering and receiving official transcripts and submitting their complete application.

Important Deadlines and Timeline Requirements That Must Be Followed

______ 5+ Weeks Before the Application Closing Deadline

  • Order official transcripts from every college or university attended, including UTRGV, STC, TSC, and others, directly from the NursingCAS application under Academic History.
  • Ensure all colleges and universities attended are reported, or the application will be returned incomplete or Undelivered by NursingCAS.
  • Ensure transcripts are sent directly to NursingCAS and arrive at least 2 to 3 weeks before the application closing deadline.
  • Do not forget that UTRGV Undergraduate Admissions also must receive your ApplyTexas.org application and all official transcripts from all colleges and universities where credit was earned. This is Step 1 under When and How to Apply on the School of Nursing website.
  • Confirm that all institutions attended are correctly listed in the Academic History section.
  • Order early to avoid errors that delay Verification. Ensure there are no holds at your former institution that may delay sending your transcript, since the application deadline must still be met.
  • Do not wait for transcripts to arrive before submitting your application.
  • Do not wait to order and send transcripts to NursingCAS or UTRGV if you have courses in progress during or after the application closing deadline.
    • An Application Update must be completed in NursingCAS after the course is completed.
    • Send UTRGV another official transcript once the course is completed.
    • Provide a copy of an unofficial or official transcript to schoolofnursing@utrgv.edu if the course was completed at a different institution.

______ 2–4 Weeks Before the Application Deadline

  • Submit a complete NursingCAS application to allow time for NursingCAS to verify your application and identify any issues that you need to resolve before the application closing deadline.
  • NursingCAS typically takes 10 business days to verify an error-free and complete application, and up to 4 weeks if transcript entries are incorrect or required transcripts are missing.

______ 3 Weeks Before the Program’s Application Closing Deadline

  • Ensure NursingCAS has received all official transcripts for every college or university you attended 2 to 3 weeks before the closing deadline.
  • Confirm no institutions or coursework are missing in your full transcript entries.
  • A 14-business-day grace period is provided only to:
    • Resolve issues with a previously submitted complete application.
    • Immediately address verification problems identified by NursingCAS.
    • Obtain Verified status before the grace period ends, which is the final deadline.
    • Understand that applications receiving Verified status after the grace period will not be processed or considered. Applicants may apply for a future cycle.

Important: The 14-business-day grace period (final deadline) after the application closing deadline is not for submitting the application or starting to order official transcripts that were required by the School of Nursing to have been ordered and received by NursingCAS 2 to 3 weeks before the application closing date, and no later than the closing deadline.

Applicants may move forward to order transcripts after the application closing deadline, but they do so at their own risk, as the application may not receive Verified status within the grace period. Applicants may reapply if the Verified Status deadline is not met. Applicants will receive a notice from the School of Nursing that they did not meet the final deadline, and their application will not be processed or considered.

Prepare Before Starting CAS Application

  • ______ Review the BSN Roadmap and confirm your prerequisite classes have been completed.
    • Use TES Guide to review courses that are equivalent to UTRGV courses.
    • If you need a possible substitution, follow the required steps to request a course substitution review.
    • Generic applicants discuss possible substitutions with Academic Advising after being accepted to UTRGV. The advisor will contact the School of Nursing with your request.
    • 2nd degree BSN applicants may contact the 2nd degree BSN Program Coordinator after following the steps above.
    • See further instructions for Advising and Course Substitution.
  • ______ Confirm you meet the minimum Application and Admission Requirements.
  • ______ Review HESI requirements and FAQs.

NursingCAS Application Sections

Utilize NursingCAS’ Help Center to guide you through each application section.

Below is an overview of each application section with important tips and instructions. Ensure you read the instructions in each section of the NursingCAS application and contact NursingCAS if you have any questions.

Within CAS: Personal Information Section

  • ______ Fill all required application sections. Generic BSN and 2nd degree BSN applicants are to opt out of the questions in the Supporting Information section, as the School of Nursing does not review or require this section to be completed. RN to BSN applicants only must enter RN license information in the Supporting Information section.
  • ______ For Standardized Test Section: 2nd degree BSN and Generic BSN only, enter each HESI exam you have taken that is not expired along with your HESI scores. Biology or Chemistry HESI scores are not needed for UTRGV School of Nursing.
    • Critical Thinking HESI score will be reported under your Questions section.
    • Enter your cumulative HESI score where it states Composite.
  • ______ If applicable, enter your AP or CLEP exams in this section. An official transcript must note these credits for NursingCAS and UTRGV. Ensure that you submit your scores to UTRGV Undergraduate Admissions for evaluation. Contact Undergraduate Admissions if you have any questions.

Within NursingCAS: Academic History Section

  • ______ Fill out all areas of this section.
  • ______ For Colleges Attended, input every college or university you have ever attended, regardless of whether you completed courses that apply to the program or not.

Applications will not move forward with Verification if all institutions are not reported, and official transcripts must be received by NursingCAS for all institutions attended before the application closing deadline. The School of Nursing will consider your application incomplete and not processed if all transcripts were not received prior to the application closing deadline.

  • ______ For Colleges Attended section, all applicants must order official transcripts to be sent to NursingCAS from every college or university attended, including UTRGV, 5 weeks before the application closing deadline. Do not forget STC, TSC, and other colleges or universities attended. Transcript notations on other colleges’ transcripts cannot be utilized. Order the official transcript from the institution attended.

Important: Check with the institution you attended, as it may use National Student Clearinghouse or another transcript service. For example, STC and TSC currently use National Student Clearinghouse.

  • ______ Review how to order transcripts for NursingCAS. Confirm they have arrived on your portal at least 2 to 3 weeks before the application closing deadline. Your application will be incomplete and will not be considered by UTRGV School of Nursing if all official transcripts are not received before the application closing deadline. This is a School of Nursing application requirement. Do not wait to order if you have any courses in progress.
  • ______ Full Transcript Entry is required by UTRGV School of Nursing. Input every course as it appears on that institution’s transcript exactly. Enter all courses you took at all colleges and universities. Not completing this accurately and completely will cause your application not to be Verified. Applicants may pay for a service that NursingCAS offers to do this, but this is optional and still must be completed before the application closing deadline.

Not completing transcript entry accurately will cause your application to be Undelivered. Correct all issues before the application closing deadline.

  • ______ Ensure that you accurately indicate repeated classes.
  • ______ For Standardized Tests, input all valid HESI exam dates and scores. Your highest valid HESI score will be considered from the score sheets submitted under the Documents tab. Scores must not be more than one year old, or your application will be incomplete and not considered. Upload score sheets under the Documents tab. HESI should not be taken more than twice in each academic year, September 1 through August 31. Late scores are not accepted. Applicants must review HESI requirements on the School of Nursing website for more details.

Within CAS: Supporting Information

  • ______ 2nd degree and Generic applicants are to opt out of all 3 sections. This section is not reviewed or considered by UTRGV. RN to BSN applicants complete RN license information only.

Within NursingCAS: Program Materials Section
