Supplemental Instruction Request Form

Please fill out the form bellow to submit a request for Supplemental Instruction (SI) services to be offered in your class. Services will be provided based on the order the request is received, the course meeting program requirements (SI Instruction is intended for high-failure rate courses), and pending the availability of funds. Please submit requests no later than 6 weeks before the start of the semester.
