Documentation Requirements
Prior to the initial intake, each student is required to provide documentation of his/her disability. Appointments may be rescheduled for students who do not submit documentation prior to the scheduled initial appointment.
The documentation should include an evaluation by appropriately licensed professional experienced in the diagnosis and treatment of the student’s disability. For multiple disabilities, various sources of documentation may be required. The documentation must make evident the current impact of the disability as it relates to the accommodations requested and include a description of any or all functional limitations.

Note: Recent documentation (within the last three years) is preferred.
Some examples of documentation that students can provide are:
- Full and Individual Evaluation (FIE)
- Medical documentation
- Psychological evaluation
- Letter from qualified professional providing treatment for disability
- Reports from the Veterans Administration
Additional documentation may be requested to support provision of accommodations and services. If a student is asked to provide information but does not do so within 30 days of UTRGV’s request, the request for accommodation will be cancelled. However, the student may file a new request once the requested documentation has been provided to SAS.
SAS uses the information obtained through an interactive interview, and documentation of disability to help determine which accommodations or supports may be needed.
