UTRGV Forms

The following forms (PDF) are available for you to download and complete for your financial aid requirements. Completed forms can be submitted in person at U Central or by fax or scanned and sent as an attachment at Ask Rio or finaid@utrgv.edu.

UTRGV Verification forms can be submitted via the Strategic Enrollment Document Central. For instructions please watch this video.

  • Once students file a graduation application, the Degree Audit team will complete a preliminary degree audit using the Degree Works system.
  • The purpose of completing a degree audit is to verify that students are on track to meet degree requirements for the semester they plan to graduate.
  • The graduation status will be sent to the students’ UTRGV email.
  • If the audit is showing deficiencies, students are encouraged to meet with their faculty/academic advisor to identify best course of action to clear deficiencies.
  • Once grades post at the end of each semester, the Degree Audit team will complete a final degree audit to verify that degree requirements have been completed.
  • If the final degree audit is showing deficiencies, students will be notified via UTRGV email and will be required to complete a Transfer Graduation Date form if degree requirements will be completed in a subsequent semester.
  • While the Degree Audit team checks degree audits for degree completion periodically and notifies students of deficiencies, it is ultimately the student's responsibility to register for and successfully complete degree requirements to graduate.
  • Graduation is the completion of all degree requirements as recorded on the official transcript.
  • Commencement is the ceremony that celebrates the completion of a degree. For more information on Commencement participation, please visit utrgv.edu/commencement.
  • Applying for graduation is required whether or not you attend the commencement ceremony.  Participation in the commencement ceremony does not imply that you have officially graduated.
  • Degree works can be accessed through ASSIST.
  • Degree Works provides information on your progress to graduation.
  • Degree Works allows direct information on pending requirements through hyperlinks to catalog information and course offerings.
  • The Degree Audit team uses the Degree Works system to verify if degree requirements are completed.
  • Review your degree audit in Degree Works. If requirements are still pending, meet with your Academic Advisor to discuss your options to meet all pending graduation requirements.
  • In extreme circumstances, a student wishing permission to deviate in any way from degree requirements must have permission from the department chair and the dean of your college, at a minimum.
