Create an Online Survey
YOU MUST OBTAIN IRB APPROVAL/DETERMINATION PRIOR TO DISTRIBUTING YOUR SURVEY.
DISTRIBUTING ANY SURVEYS WITHOUT IRB APPROVAL/DETERMINATION IS AGAINST UTRGV POLICY.
All human subjects research that includes an online survey must be conducted using Qualtrics survey software. Access to this software is provided by UTRGV and managed by UTRGV's IT department. This service can be used to create, manage, distribute and analyze surveys.

Tips
- Faculty have access to Qualtrics on their MyUTRGV account by clicking on the survey icon.
- Students and staff must request access by submitting a ServiceNow Get Access Request. Please log into your MyUTRGV account, click on ServiceNow and fill out and submit the Get Access request form.
- The first page of the online survey should have a statement introducing the purpose of the research, details on the duration and content of the survey, and information on how the data will be treated.
- Respondents should be able to skip questions, or at a minimum have an option to select if they do not want to answer the question (e.g., “I do not wish to answer this question”).
- Qualtrics has a default setting to record information from respondents based on location, which could potentially identify a respondent. If you are conducting an anonymous online survey, you must change this setting and confirm in your application on the Recruitment tab.
